Millitary medal index card

I have the above for my great uncle who was injured during WW1 and then taken as a POW. On repatriation, he was taken to King George hospital in London.

On the medal card, I understand most of the terms but under "cause of discharge" (12.12.18 ), it states "392(XV1)KR2131 W.

Under this, it has a blank for "address of applicant" but under "action taken" it states, "List D/A/635.

Very many thanks. The links are very useful.

I will follow up the Silver war badge bit. he was in the Highland Light Infantry (HLI on card) so don't think the D/A is connected to this.

Still not sure what the remainder of the letters eg W mean.

Are there any records held of hospital discharges from King George that I could follow up?
